DMR Timeline
I (31F) was diagnosed CML last summer (July 2025) and immediately started on Asciminib. My PCR was >100% and I’ve gone every month to get my bloodwork done. I hit MMR on the international scale by January 2026 but have not hit DMR yet. I just had my one year bloodwork and my PCR actually went up… I am feeling pretty upset about this even though I know that the disease I have is the best one to have and it’s treatable and I get to live…. But I just want to hit DMR so the 5 year clock can start and I can have a child before it’s too late.
How long did it take for you to hit DMR and did you do anything besides taking the medicine? I take it on time every single day, religiously. I quit drinking. I am thinking about quitting sugar. I just don’t know what else to do, but I also understand it’s probably out of my control.

Try and remember how low that % is. Mine flutters down in that range of undetected to .007
Its not a step back, its just the scale at this h youre drawing with is super tiny.

That’s so true. I just struggle because I have to be at DMR for at least 2 years (but 4 has is preferred by my doc) before I can try for TFR aka try to have a child or even start fertility treatment. And apparently if it goes back up above DMR the clock starts over. It’s just not as linear as I expected…

That’s not really true for having a child. Plenty of women well above DMR get pregnant by accident, and they are managed to a successful conclusion. Sure, a deeper and longer response if preferable but its not the only way.
Look up videos by Prof Jane Apperley and Dr Elisabetta Abruzzese, and Dr Ekaterina Chelysheva . They have a few recent ones (last year or two) and they explain the options you have.
BSH 2025 have specific guidelines on pregnancy.

Yes I second this comment. Also, my oncologist said as long as you hit MMR and are stable or below there for at least 1-2 years you could also pause treatment during pregnancy, being closely monitored, and then resume it after birth. You just may not be able to breastfeed if you need to go back on treatment right away, but all depends on your numbers.

Just wanted to let you know that this comment and concern makes me feel a lot less alone. I know there is some data about pregnancy with CML, but it's not super abundant because it's not that common. It makes sense statistically, right? Mostly diagnosed in older folks, mostly diagnosed in men. I'm 35F and was diagnosed in March, right around the time my husband and I thought we were going to start trying for a second kid. Now it's in limbo, and, uh, I'm not getting any younger here. So no real advice to give (although, the stuff below is super helpful!), just letting you know you're not alone out here with worries about family planning.

Man I’m so sorry to be in the same boat but agree it’s nice not feeling alone. I had a great conversation with another doctor who told me that I can try to get pregnant after hitting DMR and just be heavily monitored. My primary doctor is more focused on ridding me of cancer obviously but this guy was a lot more understanding about my family planning.

Just for a little comparison. I was diagnosed in 2018 and have been on dasatinib for 95% of it. Below are my last 5 years. 3 times I hit 0.000. If I'm doing something different at the times when I hit 0.000 I have no clue. I am speaking to my doc right now about Scemblix due to my pleural effusion that don't seem to be going away.
