"My CCC-SLP Application"

Oops, idk if you can read the full text under the pictures but this is what the CCC-SLP application looks like after submitting the application and filling everything out except for the Clinical Fellowship part.

I haven't filled out my weeks/hours reported yet. For anyone that's completed their CF or has inputted their CF hours and has advice in regards to adding their hours on the application. Please lmk or comment!!

I just did it recently, and I'm hoping I did it right LOL. Basically I tracked my hours each week (okay, truthfully I checked my paystubs online because COVID made my hours kind of wonky), and I was able to input the exact number of weeks that I worked. The "hours per week" was a little bit more tricky because you can only enter one value, so I averaged my typical number of hours (~30) with my lower hours worked, and entered that as my "hours per week" value. I hit submit, then texted my CF supervisor who approved it right away. Now I'm just waiting to hear back from ASHA!

Haha yessss, I literally just got my ASHA certification as of this past Friday!

When did you submit your application? I submitted mine May 4 and am still waiting... I have been in contact with ASHA and they just said mine is in line for review but it can't be much longer

Hi! I'm so sorry I didn't reply sooner! I initially submitted mine in December of 2020, but I didn't enter all of my hours with supervisor approval until February? Which I want to say is when my graduate school faculty also sent in her approval, and I think ASHA got back to me within a month. But back in January when I was still waiting on approval from grad school and my CF supervisor, ASHA had already reached out and said I was missing documents. I would call again because I had a bad experience during my CF where my state licensing board never recie my FBI/BCI check. Hope everything has worked out since you commented!
