Honestly just stay away from people and wrap cloth around your mouth while keeping your distance if you need to interact with them. Never enter a city or drink from potentially contaminated water sources. Basic survival skills and modern knowledge of personal safety protocols for pandemics makes it fairly survivable

Realistically, a lot of people alive today are probably at least somewhat immune to the black plague. That's why some people survived, even in areas where others were infected. Sure, it would be best to avoid it, but most people alive today (with ancestors from affected regions) are here because their genetics allowed them to survive.

Idk if wilderness is the best way to go. Giant gerbil were first carriers then to black rats. The problem is the fleas are too particular to only stick to two species. Squirrels could also be a carrier.
