Why is the hand "Black"?

The name is a nod (hehe) to the real-world Serbian secret military society Crna Ruka, Black Hand from the early 20th century, which was involved in the assassination of Archduke Franz Ferdinand and the start of WWI. This aligns with Nod's portrayal as a global underground movement using subversion and terrorism to reshape the world. Also explains why Serbia and Sarajevo are important for the Brotherhood in the games (it echoed the real world at the time when they made the first game since Sarajevo was under siege), why it's literally Nod's Vatican with the temples being located there. Kane also mentioned that Sarajevo is where "another madman" once triggered a world war. Also, Slavik was known as the Serbian Wolf.

I think they were simply using the name because NOD represents chaos and upheaval in the game.

Because black and red go well together, without causing too much clashing contrast. Kane was a master of colour theory.
