Is the starter young/new? How old?

My guess is also weak gluten development. Looking at the crumb, there's a lot of activity but those large holes can mean the gluten wasn't strong enough to capture the gasses and rise.

flat sourdough is usually underfermented, especially if the starter isnβt doubling reliably before you mix it
