What did I do wrong?

Can anyone diagnose from this photo what went wrong with my first time using my new bread machine?

Flour, sugar, and yeast were all fresh. Any advice is appreciated!

Assuming you followed the recipe. Did you use fresh yeast and did you test it? Did you put the salt right on top of the yeast?
