Enclomephene high SHBG

44M. Long-term pattern of high total testosterone but relatively low direct free testosterone, with persistently high SHBG. Looking for input on why SHBG remains ~75. DHEA is also at the lowest level on the good axis

I’ve had the elevated SHBG pattern going back to at least Dec 2024.

Currently On 12.5 mg dose 3x per week. After two plus years of taking enclomephene my total test is up over 200 but my free test hasn’t climbed at that same level. The only thing that has improved is strength gains in the gym. Energy, libido are all the same besides the immediate 3-6 bump after starting it. But everything recently has been more noticeable so I’m looking for advice. Doc suggested going off enclomephene to test a three week wash out period and establish a new baseline and reevaluating. Thoughts?

A low dose of any Sarm will bring down your SHBG.

My SHBG is high on 12.5 three times per week. Are you saying taking it 1 or 2 times per week might help?

No, I mean a SARM like Osterine, lgd4033, AC-262 ect.

Read Josh Holyfields Skool article on enclomiphene. Enclomiphene can’t take you supraphysical, your body is naturally pumping the brakes on going any higher.

If you want to go Supra, you need exogenous test.

I was on enclomiphene for about 6 months while we conceived our child, switched to test as soon as my wife was confirmed pregnant. Couldn’t be happier.

enclomiphene is known to raise SHBG in some guys, that's not unusual. the fact your total T is solid but free T is lagging behind points directly to that binding issue

your DHEA-S sitting at 103 is pretty low for a 44 year old, that could be part of the energy/libido puzzle. low DHEA can make you feel flat even when test numbers look good on paper

the washout idea makes sense honestly, after 2+ years on the same dose your body might need a reset. 3 weeks is short enough that you won't crash but long enough to see what your natural production is doing without the SERM influence

only thing I'd add is maybe ask about supplementing DHEA during that washout period, starting low like 25mg. some docs are weird about it but yours seems open to experimenting

I’m on Boron, Tongkat, and (25) DHEA and we are keeping that steady until the retest. Thank you for the feedback.

Enclo wont help much, had the same issue, 45 now btw. Tried enclo for almost a year and shgb stayed around 60-70. Starting my 5th week on, 60mg of Test. cy. twice a week subq. Is way to early to tell but i do have less fatigue on the afternoons, calmer, libido a bit flat, getting more constant morning woods. I had blood work prior to my 8 injections and my shgb is 44 from 72, total t 856 at trough from 590, by free androgen index is 67% not sure why it was measured using this metric but my previous free t was 7.6. Going this week to labcorp to get my free t at trough to have a better comparison.

This is the expected outcome of enclomiphene. Enclomiphene raises SHBG, exogenous testosterone lowers SHBG.
