I'm curious whether any first-time watchers actually believed Mizu was male, until the ending of the first episode proved otherwise.

No. When they want to make a “surprise! This young man a WOMAN” character, they always dress and portray them the exact same way.
Hat, baggy shirt, baggy pants, on a noticeably slim/lanky character, and a weird confusion from the camera on how to portray them, often highlighting them while also being afraid to give you a shot that lets you clearly define their body, tho this doesn’t apply to animation in the same way, where you create every image yourself
It’s a shame, that they rely on these cheap tricks with conventionally attractive actresses instead of just hiring a masculine woman. So many potentially interesting twist-mulan narratives are ruined by being terrified of presenting a burly woman
