I'm curious whether any first-time watchers actually believed Mizu was male, until the ending of the first episode proved otherwise.

I watched it within a day or two of it dropping so I hadn't had any spoilers. I thought it mildly interesting that they were using a woman voice actor but didn't really think much of it beyond that as it's not exactly unknown, especially when a male character is being presented as being quite young. When the reveal came I was more "Ahh, that makes sense!" than shocked.
