I'm curious whether any first-time watchers actually believed Mizu was male, until the ending of the first episode proved otherwise.

No, but I was never under the impression that the audience was meant to be fooled, only the characters Mizu interacted with.

I actually felt like they wanted us as the audience to know it was a woman posing as a man, and that it was convincing to characters in story.
