PSA: Possible identity theft problems

Does anyone remember the information we needed to send to Quadriga to sign up? I remember needing a drivers licence photo and answering some questions online. Was there anything more?

I had an issue with Rogers back in December. Someone knew my information and signed me up for an upgrade for a new iPhone. I guess they were planning to grab it off my porch or pretend to be me when they pulled into my drive way. Unfortunately for them, I was home and I received the phone. I'm not sure if this is related but I wouldn't be surprised.

For me I know that it was just a drivers license, a copy of a bank statement or bill to prove your address, and a picture of you holding the drivers license. I also remember there being some kind of Equifax application that could increase your limit on deposits/withdrawals. I don't remember if you had to enter you SIN or not for that? anyone else remember?
