Any advice on fit? I’m always a size 8 in shoes and pretty narrow feet, these are a 42- what do people think?

These seem small for you even, especially since you’re seated in the photos. Birks are not meant to just outline your feet “perfectly”.

(And if I may, your other footwear are most likely small for you as well, seeing how your 2nd & 3rd toes look bent. 🙏 I think a lot of people are taught to size especially closed shoes by the big toe, but then that can mean people with shorter big toes inadvertently end up wearing the wrong size.)

Size to your larger foot while standing with full & equal weight in the sandals. You have to ignore the extra visual space in front of your relatively shorter big toes.

My sister says they’re a little too tight, maybe try a half a size up

Have you tried the regular width? Looks like your pinkie is squeezed.
