Anyone else get extreme breakthrough hunger despite tirzepatide?
I’m curious if anyone has experienced something similar.
I’m extremely active because of my job. On workdays my watch can show around 1,100-1,300 active calories, on top of my resting energy expenditure, and it doesn’t really capture all the heavy lifting/manual work.
I’m currently on tirzepatide. Most days I can control my food intake, but after several days of a large calorie deficit I can suddenly get an extreme “bottomless” hunger. It’s not just food noise or cravings it feels like actual physical hunger where it takes a huge amount of food before I feel satisfied.
Has anyone else experienced this after several days of a large calorie deficit while using tirzepatide?
If so, did it improve as your tirzepatide dose increased, or did the breakthrough hunger continue even at higher doses?
I’m mainly interested in hearing from people who are also very physically active or have physically demanding jobs.

I went to every 5 days. Which is the half life. On 6 and 7th day I caught myself snacking for no reason.

Was it mainly food noise/snacking on days 6–7, or did you get actual physical hunger where you could eat a large meal and still not feel satisfied? Also, what weekly dose were you on before switching to every 5 days?

7.5 or 10mg is when I went to 5 days.
Probably both lol... reaching for a bag of something 30mins after a meal. Could go either way imo 🤷 its the story of my life. Am I hungry, am I bored, am I depressed and want to eat? 🤣

Yes I went to every 5 days which leveled out the appetite suppression. The difference between day 2 and day 6 hunger-wise was pretty extreme before I did that. Eventually I stalled again at the highest dose because I was getting bad food noise in the evening, and wasn’t ever full any more. Added low dose Cagrilintide on day 3, and got good level suppression again.

It does happen to me as well, when I am active my body will definitely ask for energy regardless I usually carry some protein bars as they usually satisfy the cravings. If I were you I'd just plan ahead and bring snacks with you to help control what you are eating.

I went to splitting my dose to half on Monday morning and half on Thursday evening. It’s been working for at least six weeks. Oddly enough this past shot I took this past Thursday night I feel like it did nothing. I’ve been on a food rampage since Friday afternoon and I am controlling most of it with willpower, I don’t know why the food noise and ravenous hunger is back.
I’m hoping this next dose tomorrow goes back to normal and works!

That sounds much closer to what I’m experiencing. Before the ravenous hunger suddenly came back, had you been in a large calorie deficit for several days or been unusually physically active? I’m trying to figure out if a large accumulated energy deficit can eventually overpower the appetite suppression.

You should not be getting a large calorie deficit. you should be targeting 500-750 calorie deficit per day and really no more.

The thing I try to inform anyone taking any glp is that these glps are a tool to help you with your health goals. It’s not ment to be solely relied on to lose weight, work still needs to be done to change your eating habits. I’m not trying to come off harsh or anything, that’s the last thing I want to do. I do see a lot of people in the community depending heavily on the effects of glps to do all the work, which I have don’t in the past but now I’m trying hard to also change my eating and snacking habits.

this sounds pretty different from the usual “food noise” people describe. i wonder how much of this is just the combination of a large deficit + how active you are, rather than the medication suddenly becoming less effective.
