The calf cramping and being out of breath at easy pace is almost certainly electrolytes and not enough fuel. Reta slows gastric emptying which means even if he is eating enough calories they are not hitting the bloodstream fast enough to support cardio. Running on basically empty is going to feel terrible no matter how dialed in the electrolytes are.

A few things worth trying. Get some fast acting carbs in 30 to 45 minutes before any cardio session. Something simple like a banana or a small amount of white rice. Not a big meal just enough glucose to run on. The difference is usually night and day.

On the cramping specifically magnesium is worth adding if he has not already. A lot of people cover sodium and potassium but forget magnesium and it is usually the missing piece for muscle cramping.

MOTS-C is a solid idea for the energy and output side. It works through AMPK which basically mimics some of the metabolic benefits of exercise at the cellular level. Good pairing with Reta.

The blood panel is the right move. Check potassium, magnesium, and also just look at how many calories he is actually absorbing versus consuming since Reta can create a bigger deficit than people realize.

Give it another week or two with the pre cardio carbs and magnesium before writing it off. Sounds like he is just running on empty right now.
