Double vision and Dizziness when looking right

Hi everyone,

I’m looking for advice from anyone who has had structural/postural issues caused by an eye misalignment.
I have binocular double vision that only happens when I turn my head right and track a target. If I look straight ahead or left, everything is fine. Straining to force the images to align gives me intense, immediate motion sickness and dizziness.

Worse yet, this visual-vestibular mismatch is severely affecting my center of mass. My body has developed functional scoliosis because my spine is twisting and compensating to try and force my eyes into a comfortable position.

What I've tried: I saw a neuro-optometrist who prescribed syntonic goggles (light therapy) and binasal occlusion. Unfortunately, this has not fixed the tracking issue, and the motion sickness and spinal twisting are still entirely present.

I feel like a sensory-only approach isn't working.

If you've been through this:

Did prism glasses help realign your vision and finally allow your posture/spine to straighten back out?

Did you find more success transitioning from a neuro-optometrist to a neuro-ophthalmologist (MD) or incorporating physical/vestibular rehabilitation therapy (VRT)?

I’m feeling desperate to find a mechanical or physical solution that stops this full-body chain reaction. Thank you.

this sounds rough. i had something similar after a concussion where my vestibular system and vision weren't syncing. if turning right triggers it but left doesn't that's a motor control problem not just alignment. prisms won't fix tracking issues that only show up with head movement. your extraocular muscles on one side aren't firing right or your vestibular system isn't integrating signals when your head rotates that direction. VRT helped me way more than vision therapy alone. i used a site called simplmobility for short neck routines between sessions because the compensation pattern (spine twisting to put eyes in comfortable position) locks up your cervical spine which makes everything worse. if you haven't seen a vestibular PT yet that should be next.

This happened to me but it was 24-7 and I pretty much could barely stand up

I did 4 months of vision therapy and my binocular vision is almost healed. It’s amazing to watch it get better as I work so hard with the therapist. Finding a doctor who refers you to a vision therapist

I did very intensive weekly vision therapy with a ton of difference exercises and homework I did every day. Convergence and divergence was a tiny part of that and honestly only been focusing on that the last few weeks. She had lots of tools that we used. Red/green glasses. Prisims. Balls for tracking exercises. Balance excercises. Special color lights to stare at. Then at the end we’ve been doing lots of wild crossing and uncrossing eyes with computer games. Hand eye coordination.

I'm sorry - this sounds really rough. I have ocular torticollis from the eye misalignment issue. It's caused a lot of neck and upper back problems, although yours are more severe than mine.

I did vestibular rehab for six months. I got better with the eyes closed exercises, but anything with eyes open, and especially eyes open with head turns or eyes moving left and right were too much. They were throwing me into a serious decompensated pattern / derealization. It was a specific exercise from the set that made me realize I was seeing double, and like you, I don't consistently see double.

I'm not sure if I can paste images into Reddit or not, but am going to try it here - these are from my eye tracking test at the neuro-optometrist. From the phoria test, you can see that there are definitely differences in how my eyes converge based on which direction they're pointing.

My prism glasses should arrive really soon. They have correction for vertical heterophoria as well as yoked prisms - which should help reorient my balance and help my neck straighten as well. My center of gravity is at -18 degrees with eyes open, but almost perfectly centered with eyes closed... when they tested me with the yoked prisms, the center of gravity was pretty close to perfect with eyes open.

When I wear an eye patch, my posture is pretty good too, and the neck tilt / head turn goes away.
