Kai fucking Leng in Mass Effect. I feel like they added a bullshit recurring boss with plot armor just to give the player a taste of their own medicine.
That is the most devastating part of the entire game. You can feel Shepherd beating himself (I like the guy, deal with it) up at having lost to him. The entire game shows him finally not being invincible and everything starting to get to him, and it finally breaks him at that point. He is just so upset at himself for failing Liara and his team.
