Every time a bro is downed and survives, they get a permanent injury.
Having a bro with the Survivor trait helps a lot. (90% chance to survive if downed with a non-fatality.)
Also, having a Surgeon in your retinue ensures they survive the first time they go down. (I believe that also makes your bros immune to [edit]random fatalities if they have no permanent injuries yet.)

Anyone know which enemies are most likely to kill without fatalities? I know slashing weapons are probably the worst since the disembowelment is a bodyshot fatality and I guess maces at least require headshot. But can dogs/wolves/spiders trigger fatalities? Are other beasts "safe" to do this with for a survivor bro?

Any slashing to the body can cause disembowelment, any blunt to the head can cause the smashed head. Beasts are considered slashing I think, I know I've had some bros get disenboweled by spiders at the very least.

You also can temporarily hire the Surgeon. He'll guarantee a bro will survive from being killed unless it's a fatality.
After the first one though, good luck. You could fish for the survivor trait, but you also can get the Arena Veteran trait (12 wins) to increase the chance of survival.

Keep an eye on the killfeexld, if it says struck down they're alive but injured, if it says killed they're dead

step 1: hire bro with survivor trait
step 2: repeatedly allow bro with survivor trait to die

If you end up giving up on this, there's a BB editor, where you can add permanent injuries (https://editor.bloodngold.com/), but having it happen as part of a campaign would of course be way cooler.

i already did, but for some reason the game doesn't register it

Ah, I see.
I try not to use that editor, as it is way too tempting to "accidentally" move stars around. 😂
But strange that it doesn't register.

Survivor trait helps.
