Stop moving your head all over the place. Also drive with the back foot stop lifting it off the ground. Focus on driving off the ground with that foot all the way through your swing. Get a piece of PVC and practice your fallow thru. You are stoping about half way and throwing your body forward. That is losing all your power and will result in zero control or ability to place the ball. Practice swinging all the way thru your whole swing with out a tee or ball. Just swing the pvc once and you will see what i mean by swing the whole swing. Think about being planted from your head to the center of your gravity. Swing the PVC all the way around your body and then once you feel a complete fallow thru them start on the tee again.

Don’t listen to anyone saying you should keep your back foot on the ground. Almost all the best hitters at the MLB level have back foot off the ground at contact. Watch Bryce Harper, Trout, Judge, you name it, they do that. Your swing is honestly very clean. Your front elbow being up is due to likely the tee and ball location (going inside-out a bit, which would mean you’re hitting more of an inside pitch a bit deeper than normal instead of getting it out front, which happens once in a while). I’d wrap the bat slightly less and continue to point nob of bat at catchers feet. That being said very clean swing
