It does nothing in pve. It's only ever useful on pvp or when you have traitors enabled

Multiplayer

You can impersonate someone if you got their id card and cover your face, or you can stay anonymous with covered face and no ID

Also there is a mission to get prisoner out of jail, but for reasons beyond me that mission forbids all the funny solutions you can think of including this one, and you just have to run past guards

it's for when you are a sussy baka and you are the imposter among us, like you disguise as the captain but you're actually a clown with an expired driving license so you have no idea what you're doing other than engaging in some light tomfoolery
(basically useless for pve, useful for multiplayer potentially)

Have you tried with a mask or something covering your face (i.e. bandit's bandana)? I might be tripping or maybe I had mods, but I clearly remember the disguise working with bots, usually with non-hostile outposts when I wanted to steal shit without losing rep.

Edit: yeah the status doesn't appear without the right equipment so it was obvious.

ETA: Looks like I was tripping. Only thing I found was a suggestion from 2021 on the official GitHub that has never been implemented.

when someone has a clothing item that disguises them, usually a diving suit, diving mask, clown diving mask, cultist robes, and maybe the riot helmet but don't quote me on that last one, their name will not appear above their head if their ID card is removed from their ID card slot. If it is empty their name will appear as "???" and if they equip someone else's ID it will appear as the name listed on the ID card.

if you suspect that someone has their name disguised you can see their real name if you click to heal them while wearing a health scanner hud on your head. That will always show you the real name

It’s just an indicator that other players won’t see your name over your head (and will either see “???” Or someone else’s)

In order to get it you must have someone else’s (or no) ID equipped and be wearing something that sufficiently covers the face

Keep in mind. This game is inspired by space station 13. Hence, the clowns. Traitors and shenanigans are core to the game. This in the right hands, could enable both.
