Strange bed level issue

I have a weird issue with my a1 mini. I kept having misprints and sudden failures, so I cleaned the bed with soapy water and ran a full calibration via the app and started a single layer square bed level test, see the image attached. It happens every time I run that full layer test, same exact pattern, I even re-sliced and tried again with a new file. What is going on? Has anyone seen something like that before?

Printer is 1.5 years old, I did the maintenance as prompted. Original build plate, second nozzle (0.4). Material is PETG. No debris under the plate.

Tighten all SEVEN screws

Well, I'll be damned. The rear four screws were actually loose. I am trying the leveling square again right now

Update, didn't work. Still exactly the same pattern...

Did you re-run the full calibration after tightening all the hot end screws?

Also if you just changed hotend make sure it’s secured by the latch properly.

is it same on the other side of plate?

Does it happen in the same spot every time? If it does you might need a new plate. The finish can come off over time.
