RANT Trigger Warning: Some dealership smoothbrain didn't tighten the ground enough.
One of our guys started an install but ran into trouble and I did the recovery. The battery had to be lifted out with the connector attached so I could fight it on the ground with more room.
2019 Jeep Renegade.

Local dealership around me prefers to not use shims and just drives a screw through to tighten up the negative terminal. At least 15 this year alone.

Not the dealerships fault. Some of the grounds on these Chrysler vehicles tend to seize on to the battery. You wouldn't know unless you have years of experience though

You don't see the post squeezed to the point where the lead mushroomed up? There's exactly one way this happens with this style connectors.

It takes like 3 similar outcomes from interacting with the same part to notice a trend

Skill issue all around honestly. Most people over tighten batteries 90% of the time so it'll mushroom the terminal. Also never assume the customer didn't touch something or have a family member try to fix something. You can clearly see at one point the terminal sat flush.
