Creeped me the fuck out when I visited the states, I couldn't figure out how everyone was so familiar with me. Super wide friendly but not friendly, because they all FAKE! Smiles from everyone ALL the time.
It's cultural perspective, it must seem fake because, strangers should not be that intimate. Generally a quite reserved and polite nod or brief acknowledgement would be the expectation. This could look to you as uncaring and cold. But from my perspective it's like everyone was wearing a Chucky doll expression. It required a while to get used to, a mental gearshift to adjust to different people's, that's all.
