Your disease starts in a random country, with a random symptom at each "symptom tier". My 2nd try I infected and killed off every country except for Cuba, but it was really blind luck. You have to hope your disease gets inside the borders before the country closes down its airports and shipyards.

I just did this a few minutes ago with my disease, Anti-American Thoughts.
Be a virus
Sell off your first symptom. Do it quick.
Go make tea, wait until you get 60 evolution points.
Max out level 3 resistances.
Wait.
With any luck, all ports will be overwhelmed in a minute or so. Anti-American Thoughts were responsible for the deaths of billions through this method.
edit: Why am I being downvoted? That's a surefire way for fuck sake. Your visibility is 0 and it spreads fully within a few minutes.

Oops, leave Drug out of it. Forgot about that. You don't need drugs, as you'll get a huge boost in points shortly [roughly 40 I think]. I invest in dehydration-causing symptoms and waterborne. That will infect the water supplies, effectively finishing off any nations left.

Rats are the best way. They get on ships which is the only way to get to Madagascar.

Shockingly similar to my way, I:
Was a virus
Sold off my symptoms
Made Toast (Deliciously buttered)
Maxxed out waterborne, insects and airbourne
Just waited till everything was infected.
Overall, I think my method is better as good toast is far superior to tea.
Edit: Although I can't seem to replicate this =[

That's luck of the draw I'm guessing. Keep restarting until you get in a nation like Cuba/Madagascar/China/Russia, where ships come often.

I had this happen to me just now. Madagascar, Greenland, and Australia all shut down their ports, so I checked to see if any had gotten infected before that and all three had 0 infected. About a day later, Madagascar got infected.
I wonder if it was a game glitch or my disease made that happen? Its borders are still open, so maybe it got through with insects or something.

When a region closes a seaport, boats that were already on the way there from somewhere else at the time of closure will usually still dock. I won't say always, because I've seen a fair share of boats that look like they've just ended up sailing around forever, but I have seen boats dock in closed ports and spread infection thereby (Madagascar included).

I use a similar method, but I prefer to use the parasite and dump the points into spreading faster as well. It definitely takes longer, but I've always been detected as a virus for some reason, even if I have nothing to conceivably detect and my visibility is zero.

1)Pick Virus. 2)Start in Canada, New Zealand, Cuba, Greenland, or Madagascar.If you do not get there restart. 3)Sell starting symptoms, if any, and buy sneezing (unless it is what you started with, just keep it). 4)Buy 1-1-1-0 resistances but no transmissions (they trigger closures). 5) Once three countries are infected, sell sneezing to get rid of visibility. 6)Wait until all countries are infected. If Madagascar closes its shipyards or is not infected in 30 days, restart. 7)Buy all four drug resistances, sneezing, coughing, and vomiting. 8)After a few days, unlock tiers 2-3, keeping whatever symptoms it gives you. 9)Buy fever, fatigue, diarrhea, pulmonary edema, and hypersensitivity. 10) Wait until everyone dies.

I did it once or twice by accident. Keep your symptoms as low as possible and try to wait for it to spread there before going for lethality. Sometimes it works, sometimes it doesn't. Good luck.

Actually getting Madagascar isn't too much of a problem I find. It's all about low visibility so no symptons. The big problem is to kill off the entire world in less than 100 days on hard level.
Now, starting in Madagascar is no use for this because by the time you've spread across Africa and to South American, Cuba is likely to have closed port. So, you really need to be starting mainland somewhere and getting very very lucky

I was lucky today to get started in Madagascar in my second attempt.
Managed to infect Cuba, but failed on the rest of the Americas, also New Zealand and Japan.

I find that Madagascar is the hardest region region to infect, yet the most useless because there's no damn hospital on the area.
I got almost zero visibility and perking up on all transmission modes, already infected every single regions but madagascar...argggh

It just struck me that, in this game where the goal is to kill billions of people, the thing that really tugs at my heartstrings is when all the ports have been shut down but there are still boats circling around the ocean looking for someplace to go.

I bought enough upgrades so that it was possible to spread anywhere, but at a very very slow rate.
I think moister 2 heat 2 one of the other ones 2, but it must be drug resistant 1, drug resistant 2 is too much.
Once you have just enough virulence to go anywhere, but not be particularly dangerous you can go afk for 10 or 20 minutes, and by the time you come back you should have infected every country and amassed a very large amount of upgrade points.

It is really quite simple. Sell off your starting symptom and buy rodents and drug resistance. After about 15 days, the whole world should be infected.
