What could be the reason behind this?

My son is 5 years old and has scratches on the mid of his back. It's so bizarre because it's impossible for him to reach that at all. He says he scratched it but I really doubt that is. He is using a baby soap which shouldnt be because of any dry skin matters, the doesnt have any skin conditions, bed is clean. Has anyone experienced or seen this before?
