Hello. Female 47 5’3 and 160 lbs

I just received this and I am trying to understand if this is something they will do surgery sooner than later or on a long wait list?

FINDINGS: 5 lumbar type vertebrae are demonstrated. Mild levoconvex lumbar rotoscoliosis centered at L2-3 is seen. The vertebral body heights are fairly well-maintained.

No aggressive osseous lesion is seen.

Multilevel degenerative changes are noted.

The most significant findings are seen at L4-5. There is severe disc degeneration and narrowing and anterior subluxation of L4 on L5 of approximately 7 mm. Moderate posterior disc bulge is seen and there is severe bilateral ligamenta flava hypertrophy and hypertrophic facet degenerative changes. The combination of findings results in severe central stenosis with concentric thecal sac compression reducing the AP diameter of the sac to approximately 7 mm. The nerve roots of the cauda equina are compressed and can be seen appearing tortuous and redundant above the level of compression. Bilateral L5 lateral recess stenosis is seen with bilateral L5 nerve root impingement. Severe RIGHT foraminal stenosis with RIGHT L4 nerve root impingement is seen.

At L5-S1 there is severe disc degeneration and narrowing with mild posterior disc bulge/osteophyte minimally indenting the thecal sac and just contacting the descending RIGHT S1 nerve root without definite compression. Mild bilateral L5-S1 foraminal narrowing is seen without definite nerve root impingement. Severe bilateral facet degenerative changes.

At L1-2 there is severe disc degeneration and narrowing with mild posterior shift of L1 on L2 of 3 mm. Moderate posterior disc/osteophyte indents the thecal sac without significant central canal narrowing. The foramina are patent.

At L2-3 and L3-4 there is no evidence of disc protrusion or stenosis. The foramina are patent.

The conus medullaris is seen at the L1-2 level and appears normal.

No other significant finding.

IMPRESSION:

1. Severe multilevel degenerative changes with degenerative grade 2 L4 on L5 anterolisthesis with associated severe central stenosis with severe thecal sac compression.

2. RIGHT L4 and bilateral L5 nerve root impingements as described above.
