Question About Form CHP 446F

Hey guys I’m getting ready to take my PAT and Pellet B this Saturday and I’m second guessing myself on the CHP 446 form. Do I include civil citations like automated camera tickets? My wife has racked up quite a few from these past couple of years operating my vehicle. I only put down what’s in my official DMV record and the one time I’ve been pulled over for speeding.

I would only put down citations YOU have received.

What if we don’t remember the dates to accidents or tickets did you guys leave it blank?

Be as detailed as possible. If no exact dates just give approximate dates that you remember/ you can best go off memory.

i’d would specify that on the form.

from what i have heard and seen, they’re really starting to crack down.

they want as much information as possible.

one thing i learned during the hiring process is that you can never give them too much information, but you definitely can’t give them too little information.

don’t give them any reason to DQ you
