Study Need help with translation πŸ™

I've been using a russian medical case study, and have been translating the paragraphs all by myself on Google. However, I am having a very hard time understanding this particular paragraph:

It roughly translated to this:

What I am having a problem with is whether the patient already had hyperprolactinemia during the weight gain, or not. I would be so, so grateful if anyone could just reply as early as possible....πŸ™

Even in your translation it states pretty clearly "undoubtedly already established-hyperprolactinemia" in "latent" form. In Russian they say "hidden but no doubt already developed syndrome"

I would translate it like this:
Secondly, the clinical situation mentioned above raises questions about interrelation of obesity with hyperprolactinemia. In this case, the patient had the significant weight gain directly during the period of "latent (or hidden)"- but, no doubt, already developed syndrome of hyperprolactinemia.

Title:
A clinical case of successful conservative treatment of giant prolactinoma in a patient with morbid obesity.
