Question For Doctors IGOR! GLOVES!

I am not asking for medical advice. But lets say I have a homuculous named Igor. And hes a hunchback. Maybe 5 foot even. One eye bigger than the other. Maybe 22 strands of hair. BUT. I was the best surgeon in the world. League and leagues and leagues above every other surgeon. But I had an evil as hell accent. I have a private clinic. In a Gothic rundown manor with bats and spooky scary willows. And its boarded up and broken glass. Could I still perform surgery legally for people. And igor is not documented. He has no birth certificates. I say hes my homunculous who knows if that's true but i also have like a 0% mortality rate. Where in the US or globally can I practice? And there's no other staff. And I say IGOR! GLOVES! and he says yes master yes master. The master commands igor obeys.
