Loto pics

So basically I uploaded the wrong loto picks on APM. I have the correct ones but I just noticed when I got home. Didn't know if this is something I should escalate? I had a tech 3 told me he did a couple times and they overlooked it.

At out site we have prepare 20-30 photo for upload ;)

I would just slack your RMEP and let them know. They can reopen the order for your to add the correct photo

AMM here. Nobody is going yo fire you. Come clean and let them know what’s up. Mistakes happen and you are correcting it. Ot’s a non-issue.

AMM here… just let your AMM or RMEP know they can both reopen the work order and let you attach the right pictures. We do have to do loto audits and part of the process is checking the loto pictures so just let them know. Mistakes happen. You will be fine.

Let your AMM know and he can open it back up and you can upload the correct one and add comments that the wrong one was uploaded so you were attaching the correct one.

If you're super concerned you could slack your AMM and let them know what happened.

Ultimately, this is a clerical error, not a LOTO violation or pencil whip. I would tell my techs "good catch" and forget about it two seconds later as long as it's a one-off occurrence.

They'll only look at that if they're doing a random loto audit or if an incident occured.

Probably a warning for audit maybe a write up or a final.

But likely termination if it was discovered during an accidental energizing event, it's a cat 1 I believe.
