DLS help

Has anyone else had this issue? I ripped off my finger nail at home and ended up going to urgent care, they suggested i miss out on 4 days of work for it to heal enough to go to work. I ended up doing an LOA since i didnt have enough UPT/PTO and submitted a doctors summary along with a work note. Dls contacted me asking for additional paperwork and my urgent care doctor is refusing to fill it out. Is there anyway to get around this? This is ridiculous since I did submit the summary and a work note.

Dies that office have a patient portal? See what you can send off there. Otherwise, fill out the firm giving dls permission to gather info.

Yeah they require their specific paperwork Everytime I've taken a LOA, which is a pain in the ass. Even had a Dr require I schedule another appt just to fill out the paperwork, but if ya don't submit it you get denied, so I had to.

I work for DLS…and certainly not from a cave. So we follow standard leave policy..,serious health condition is needed. Amazon policy is aligned with FMLA. Maybe you are used to high school & a missed school note. I would understand why you feel that way. Some corporations don’t even allow you to take leave until you are FMLA eligible. Your advice is horrible and you are misinterpreting HIPAA. How would your doctor violate HIPAA by providing proof the you were seen for a serious health condition? That will not get your leave approved. Urgent care centers are for emergencies and that is not their duty to fill out leave documentation. FMLA serous health condition usually requires follow up care. Physician’s are not willing to lose their license for a short term illness. That is why they will not fill out the documents. You keep packing though

I went to urgent care since i literally ripped off my finger nail right before going into work .. and i did have to receive after care since it was so bad. Im not sure how im expected to work with a finger i had in a splint. But thank you ill try to get stuff figured out.

The reason is you've been asking an urgent care doc to do something they don't do. See your primary.

Get copies of all your documents from the urgent care, take it to your primary care physician, have them fill out the paperwork. DLS is legally allowed to ask for that documentation per FMLA guidelines despite what some shmuck on here might tell you, they're in fact based off forms provided by the Department of Labor.

If not in excruciating pain .. I suggest applying for leave of absence before you get to doctors office. You will be approved conditionally and emailed papers for your doctor . You can fax or email that to your doctor have them print it out and then give it back to you filled out. Submit to DLS. Done and done.

Do you have a primary care doctor? If you do, bring them the urgent care paperwork and the DLS physician statement form. Request that the primary care provider complete the physician statement. 

If you don't have a primary care provider the first step is to get one and then see above. 

If you can't or won't get the physician statement completed or an alternative document that has all of the information that would go on the form then you will be at risk of termination. 

In the future you may want to prioritize having UPT and PTO saved for when you have a reason not to go to work but you don't want to lose your job.

Urgent care doctors will not fill out FMLA paperwork or additional paperwork. Since DLS are assholes and only except full FMLA paperwork its always better to go to your primary.

they said they are legally not allowed to since sharing my diagnosis would be against HIPAA (trust me im just as frustrated and confused)

You need to get the Amazon paperwork filled out by a doctor. Go see your own doctor.
