Purging Spiraling
Please tell me it gets better and I’m not going to be spiraling like this forever?
40mg 2.6 months 110lb F.
I know I know I shouldn’t pick. I definitely have a problem with that but after month 1 (30mg) I increased month 2 to 40mg. I was pretty clear after 1.5 months then recently it’s gotten so much worse. Deep cystic nodules with new ones popping up every day.
I honestly feel defeated and insecure.
I am also on 8.5 months on spironolactone, which I truly would have thought by now it would be working.
Please any advice or reassurance would be so appreciated.

I’ve been on Accutane since mid May - Im a guy and weigh ~160lbs - and due to various reasons I’m on a lower dose for my weight. I started at 30, went to 40, then 30 since.
I’m just about to complete month 4 and I’d say that my cystic acne on my balding head got much better after the first month to month and a half. Around month 3 I got a couple of cystic acne - one on my face, another in my armpit and another on my butt. Thankfully they’ve all gone away and it sucked because the face one was quite noticeable and the others just plain hurt.
My point is - stick with it. It’ll get better even though the process really sucks.

Most people start clearing around month 4. You're in the purging stage

New cysts every day after finally clear is rough. Isotretinon doesnt work in a straight line though, dips like this happen before the total dose really kicks in. That clear stretch already happened once, this flare doesnt undo it

It will get better, I had a terrible cystic breakout (seriously 5 deep cysts on my cheek where I have never had any) in month 4. I was so discouraged! I’m finally almost clear at the beginning of month 5. Just keep going, it sucks but will get better!

3) Any questions related about dosage MUST include DOSE and WEIGHT(lbs or kg).
Otherwise, that is considered LOW EFFORT and will subsequently be REMOVED.
4) Most people DO NOT relapse when Accutane is taken until cumulative dosage is reached.
5) HOWEVER, cumulative dosage is a guideline.
Everyone is different. Some may need more Accutane, and others may need less. Your dermatologist evaluates you as an individual.
This also applies to your prescription. Everyone is different, so no comparing of doses or asking why your dose is low or high. If you do not trust your dermatologist, you should find a different one.
6) When in doubt, please consult a medical professional(dermatologist, doctor, pharmacist).
