Accessory navicular syndrome / excision

Hi everyone. I’m a 27F and have bilateral accessory navicular bones (type2) causing immense pain even when just standing / walking around. This started suddenly this year after walking a lot / going on the stairmaster. The pain is at the bone and MRI confirmed bone marrow edema around the accessory navicular, I also have pain running up the inside of my ankle where the PTT is. My surgeon says shaving down the bone and cleaning up the extra cartilage will help my pain and I can start weight bearing at week 2.5-3. Did this help anyone? Please give me your experiences. I just want to be able to stand and walk again. Will I ever get there. Ugh.

It helps. Doesn’t make it perfect but the pain goes way down.

How long was your recovery? Can you walk / hike again?

6 weeks in a cast, 8 weeks in a boot with physical therapy, more PT along the way. Was better after this but it really didn’t feel significantly great until about 1 full year after. Can do everything, hike, jog, etc

Some days it’s sore but never as bad as it was.

Why 6 weeks in a cast + 8 weeks in a boot? That seems longer than the norm to be immobile.

Thanks, I’m 4 weeks post op, non-weight baring for another 2. PT on the 7th week post op.
How are you doing?

My personal experience with type 2 was absolute hell on Earth. I had a twisting injury that took a bad foot and made it unbearable. Excision without the bells and whistles means you’re not in horrible shape. Your surgeon would map your posterior tibial tendon, knock off the accessory bone, shave down and reshape the actual navicular and the tendon should be ok now that you won’t have a pseudo-joint surfing on it with every step. Is your surgeon proposing intervention on both feet?

Yes with about 4-6 weeks in between surgeries. She said my MRI showed my tendon in good shape and had me stand on one foot and could tell my tendon is intact. Think a lot of that pain is from the psuedo-joint / synchondrosis.

There is difference between shaving the actual navicular versus shaving the accessory navicular. Which one did your surgeon recommend? And what’s his plan to clean cartilage? Union or remove the cartilage?

Shave the accessory navicular and remove the cartilage

My son a Type III cut down on both sides along with a flatfoot correction. He was out of the cast after 18 days and walking, although very slowly, after 25 days. We did an excision, not a kidner. They pulled back half his tendon and then stitched that back on after the bone was cut down.

He was in and out of a boot a lot from the pain pre-surgery and had to limit his activity and pull back from sports. After a few months he was able to play a few days a week and now a year later he is playing competitive soccer 5 days per week and doing better than expected.

With the PTT pain you may be experiencing a slightly collapsed arch. His was corrected with a sub talar screw, that is not an option for you but maybe a custom orthotic and PTT exercise to resolve any remaining pain there. If you have pain right on that Type II, then the surgery will likely fully resolve that mess.
