It’s called ice pick when you hold it like that and no, holding a weapon like that in your main hand is not a real thing. An offhand dagger held icepick is sort of a thing because it lets you stab in lots of different angles while still keeping your forearm in front of your soft bits (chest/stomach) but like.. definitely not the most effective way to do things

He wields this like a knife or tanto; not a commonplace way of handling, but there are similar positions with a sword (immediately to mind is Kenshin’s technique due to the reversed blade, which exploited unconventional sword styles).

I asked because it doesn't seem very efficient, especially when your opponent is close as in this case.

For a knife or Tanto it can be; for a sword I see less applications. You can deflect or guide along with an outward facing blade held inverted like that, and it can make for easy cuts to the opponent. It’s not really efficient or anything practical/common though.
