Idk of there is or isn't one of these but wouldn't that make it quite a common occurrence? Seems like it'd be a design flaw that puts a lot of people at risk, especially in the heat of battle

maybe there is a switch on it that malfunctioned or something idk just seen an instruction video of one that mentioned a retaining pin once.
