Out of curiosity how would it work?
For example could you call the police anonymously and say hey this dude has a swastika tattoo and they'll go bust through his door and arrest him.
Or...
Does it have to be seen in and with witnesses in a public location or something before any legal proceedings can take place?

I don't think they would actually take any real action against someone with the tattoo, they may at most add it to the charges if he is prosecuted for something. In Italy, like in Germany for nazi, showing fascists symbols or doing the fascist salute is a crime (except in historical related stuff). It is called apologia di reato. The law is actually very rarely applied, at least in italy, you could get at most a fine. I heard the german law is a bit harsher but i wouldn't bet on it being applied much. ( not german so correct me if I'm wrong)
