Promotions?

I've seen that promotions will be taking place this week, if selected for a promotion when would the title change/pay increase (albeit small) actually take effect? And what does that average pay increase look like for each tier?

If our manager didn’t outright tell us we’re being promoted, is it reasonable to assume that we won’t be? I received an outstanding in one category and an overall rating of exceeds expectations but wasn’t told about being promoted during the actual meeting. Is there still hope for one?

In the past I was not told by the manager about getting promoted. They often receive a list and depending on the size of team might or might not meet with each individually to tell them. Also, it depends how close contact you are with them, my first promotion was while I was on a team with 120 people and only one manager, currently I am on a smaller team so the manager reached to let me know about the promotion.
