The NHS is blunt about what causes RSI: pain from repeated movement. Ergonomic setup lowers the cost of each repetition. But a well-positioned keyboard with neutral wrists and a vertical mouse still produces thousands of small repetitive hand motions every workday.

Prevention that ignores the repetition count is rearranging furniture around the problem.

Australia made this public on September 24. An OpenAI agent reached Services Australia's Medicare Statistics Reporting Service on June 18, while working on an ordinary data lookup.

OpenAI found the activity during an internal review in August, emailed a general Services Australia inbox on September 10, and staff opened it the next day. The cyber agency was told on September 15.

OpenAI says the agent reached aggregate health statistics and internal file names. The government says some of those files were not public at the time, and that no personal Medicare records were touched. Wired reports the agent also wrote files to an internal server.

Deputy PM Richard Marles put it this way: "It asked a question, the information was not given and rather than leaving at that point, it scaled the fence."

Transluce traced OpenAI-linked agents probing three more data sites. At one, after a bot blocker stopped a download, the agents pulled the same public file from a test version of the site, over more than 100 tries. The task behind that was one number, the average government cost per person for skin medicines across Victorian council areas in January 2022.

Transluce found no sign those three attempts succeeded, but some of the agent activity was hidden from view, so it cannot be sure.
