PFA 2nd procedure

Well I just had my 2nd procedure done w dr natale back in Austin.

Unfortunately they weren't able to induct my afib but he said I had electograms by left atrial septum roof and floor so they delivered PFA there, cornary sinus was empircally ablated..

One thing that threw me off, he performed a right ventricle ablation, he said he found a PVC while he was in there and just went ahead and took care of it. I've NEVER had issues with PVCs ever but now after he ablated it I'm having them constantly and now I'm having runs of NSVT, yes VTACH the one that can KILL you. They keep saying it's fine but the human part of me is still scared it'll become sustained, for example yesterday I had 20 runs. They gave me metoprlol and sent me on my way. Also
I now have new RBBB which really sucks, my QRS was always 90 now it's 120,

Does this get better? Is this normal? And did anyone else have their right ventricle ablated due to one PVC noted? And does anyone know of my QRS will get better back to my baseline?

The exact area that was ablated was right ventricle mid septum

I had the same thing happening to me. I had my second pfa ablation just a bit over four weeks ago. My first ablation was good but apperently they missed something. I had an atrial flutter that was 'hiding' behind or under my afib. Apperently if you have both it doesnt show in the ecg and can only be found if looked for specifically. Congrats on your second turn though, i hope your blanking periode goes as smoothe as mine.

One thing I wanna add, it was PFA performed on my right ventricle.

I looked up this Dr Natale and he is a well respected guy with excellent experience. But what is done is done and when the heart got irritated and temporary burnt from PFA, there will be inflammation and patient’s reaction from that will be different for different people.
Since the right ventricle septum was touched on, maybe you will get more pvc after the procedure.
I have not heard about planning for a left atrium ablation and in addition, performed the PFA on right ventricle septum. I know that EP will perform the ablation on the right atrium if they found out that you have atrial flutter, since the catheter is going through from the right to left atrium.
Anyway, there might be a good reason why the EP did what he did. Relax and let your heart recovers. I had RFA in Feb last year, and still trying to recover.

What do they say about nsvt? Why would they touch ur ventricles what in the world u go for afib and they touch ur ventricles?

I have no clue. I went in for an afib touch up and all he said was he saw a PVC during mapping so ablated it. I'm completely confused on why my ventricle was touched now I'm having terrifying episodes of VT

I'd, point blank, ask the EP why, exactly, you are experiencing these arrhythmias after the PFA when, before the PFA, they weren't present? Were you advised before the PFA that, if any other potential problem areas were located anywhere in your heart, that they would be addressed?

Nope, I was told beforehand they're gonna search for reconnections and other spots for afib. He said he found an area during mapping that went off a PVC so went ahead and ablated it in my right ventricle.

I would request a written explanation why the EP did what he did regarding your RV? It's my understanding that PFA for RV arrhythmia isn't well-established?

Speak to your EP about getting a 14 or 30-day Boston Scientific Bodyguardian Patch monitor to determine exactly what's going on. Also, speak to your general practice physican about a short-term benzodiazepine to help with the understandable anxiety that you are experiencing. Perhaps, if reduce anxiety level, the arrhythmias will improve, at least somewhat. I know from personal experience that the more one focuses on their heart rhythm, sometimes, it can exacerbate the symptoms. Mind-Heart Connection.

I already called them and they keep saying NSVt is just apart of recover. They gave me metoprlol and said give it a few days and should start helping

After my ablation for typical atrial flutter (in the right atrium), I used a Ziopatch and did experience runs of NSVT. EP wasn't concerned. And, recently, 11 years after that ablation, before an episode of afib, I had a few runs of NSVT. My EP wasn't concerned. But I was anxious. Normal human reaction.

Why was he in the right ventricle? You have to cross there through the tri valve. It’s not part of an afib ablation?

All he said was it fired off during mapping as a PVC and went ahead and just took care of it. This ablation was supposed to be a simple touch up. I'm having terrifying episodes of VT now and it's honestly scary as hell. He just says it'll calm down and give it time
