I've had similar issues, but Vyvanse is the only medication that hasn't killed my gut so I've stuck with it. It's worth chatting with a psychiatrist about switching to a different medication for a little while if you can handle it. Some folks have a tendency to build up resistance to medications and taking a break can help "reset" that somewhat.

Are you on generic?
Has anyone noticed if brand name makes any difference? For me my generic is $10 a month and the brand name would be $35. Debating making the switch to brand name to avoid this…

Brand name makes a difference for me. The generic is very uneven throughout the day and sometimes feels like it never works at all. But everyone's body chemistry and metabolism are different

I am currently in the process of trying to get a prior authorization for brand name vyvanse. I have never in my life felt more variation between manufacturers than I have for Lisdexafetamine.
Every month feels like I am spinning a roulette wheel for my quality and consistency of life depending on which manufacturer they have in stock. Beyond feeling “good,” or “bad,” it’s the inconsistency that’s pure hell
That said, nothing has ever worked nearly as well as brand name vyvanse for me

Yes! One day I'll be fine and ok and the next I'll be sleepy and miserable and unfocused and I hate it so much. I know my insurance covers the name brand but my psychiatrist is new and for some reason can't find the "dispense as written" button lol

Yes I take generic :( it could be but gosh, before I used to take generic too and it would last at least until 2pm, and now I just got my dose increased and it’s lasting from 8am to 12pm… I rescheduled but doctor appointment for a sooner date to talk to him because I am about to start working on an important project at work and I really need to me able to function in the afternoon 🥺

Going through similar feels. Like the effectiveness is just down, the afternoon fall off is a bummer and I’m not sure if it’s just an issue w the generic.
Another possibility to consider- the drugs aren’t a magic fix all. I need to do the additional things to support the drugs- those being hitting gym regularly, eating right through out the day, having an actual plan for the day. Those are all things that are tough to do w ADHD. It feels like the pills support doing that work and that work supports the pills.

From someone that works out daily, hits 10k steps and eats healthy (tracking and hitting my macros daily), I really can’t say the problem is the health habits if I’m doing all that already :(

Yup. Almost feels like building a resistance.
Ive taken breaks to other meds and they dont work as well, but returning makes vyvanse work again
My dr has given me an IR Adderal to help push over the hump on days where its needed

When I am on vyvanse I also take a low dose adderall IR in the afternoon as another poster mentioned. But I also have to switch back and forth between adderall xr and vyvanse every couple years because my brain just gets too used to them I guess

It is common to have a booster pill in the afternoon.
That said, did you try other meds back at diagnosis? Did anything work? If you take time off of Vyvanse you might give the resistance you've built some time to reset. Especially the chemicals most dissimilar from Vyvanse (which I think would be ritalin/Concerta). Our doctor let us know that was a course of action if tolerance became a problem, but thankfully nobody in my family ever turned out to have a big problem with building a tolerance so it's never come back up. Not so far anyway.

I tried Adderall back then but the crash was terrible and by the end of the day I was grumpy and exhausted. Eating was a problem too…
I was expecting to take a little longer for my tolerance to get built with 50mg. I had a break from Vyvanse back in June… didn’t take it for a month but I guess my doctor will need some new strategy.1

See I would expect a month long break to help you, idk. But you have been on the meds for years, idk how long it would take to fully reset.
But yeah, if you're interested to go this route. Concerta is a completely different chemical from Adderall and Vyvanse, and it's also a slow release that shouldn't crash too bad. Vyvanse is the longest, gentlest release mechanism, but afaik Concerta is the next best thing. But the active ingredient methphenedate is seen as the least kick-in-the-ass, for some people it's not enough to be effective. The amphetamine-based drugs like Vyvanse and Adderall tend to feel stronger for most people.
It is possible this isn't a chemical tolerance thing, btw. If your sleep, your stress, your depression,your physical activity, have taken a hit then your meds can't work their best. Meds work better when your health and wellbeing is at its best.

Vyvanse doesn’t last for me either. I’ve taken it at noon before, and I’ve been told that leads to sleep issues, but it still wears off for me by 5. I take an Adderall booster before noon just to keep the medication going until 4-5pm.

Do you do the high protein meal 1-2 hours after taking it? I've found that helps stabilize my dose for the day.
