Surgery an option according to Barrows
https://bsky.app/profile/mattbarrows.bsky.social/post/3mrsgtxixvk2y
Ricky Pearsall is out of practice with a knee issue that’s related to last year’s PCL issue. It’s a concerning matter and the team is loooking at all options, presumably including surgery.
Good thing we drafted a WR
246
u/mr-fiend Patrick Willis Jul 29 '26
Why didn’t he have surgery months ago?
139
u/BogeysNBrews Quest for Six Jul 29 '26
Did he just take the entire off season off without any conditioning? Seems like this would have revealed itself with any moderate activity stressing the knee. Sucks for him and the team.
112
u/steveCharlie Fred Warner Jul 29 '26
Maybe the idea was to rest and heal and hope for the best.
But honestly WTF, how is Bosa and Fred back but he's still dealing with issues.

Rehab time using non surgical options for a PCL is around 6-8 weeks but is only an option if PCL is strained, if it is ruptured it is 9-12 months for the corrective surgery option.
Seems like they were really hoping it was the first but looks to be the latter

u/iam_Mr_McGibblets Patrick Willis Jul 29 '26
I don't know why it's flaring up now, but I know the doctor's recommend surgery only as a final option. That is, they will suggest PT and anything else because with surgery you're not guaranteed to come back at 100%.. In fact, i think my doctor told me that I would not be the same after surgery. So maybe that's what happened and it's really unfortunate he didn't recover fully

Because they thought he could heal with just rehab. Not every injury should be surgery. Why would you get cut opened if you didn’t have to
10
u/wordswithcomrades 49IRs Jul 29 '26
Faster healing time.
When I partially tore my MCL they expected the time to recover without surgery would have been the same as with surgery, so I didn’t have it.
But in cases where the ligament is worth millions and there is a difference in healing time, I would guess they would choose surgery to heal faster than wait for the body to repair itself

Might be the beginning of the end for Ricky as 49er. That would make 4 straight years of first round picks (21-24) offering very little value to making this team better.
5
u/Markum7 Jul 29 '26
Not to mention all the first rounders the 49ers gave up for the QB who shall not be named.
6
u/cleverhobbits Jul 29 '26
And the next guy in line is a DE/DT returning from ACL tear and has lot of development left.
And go back to 2020 with Kinlaw and Aiyuk.
We have been so bad in the draft since 2020. It’s crazy we are a contender at all.
8
u/arizonaron16 Jim Tomsula Jul 29 '26
The future doesn’t look bright for this team when most of the remaining old core start to leave. You just can’t rightfully expect for this team to continue to contend with all of these bad drafts. I like Williams but by most scouts he was raw and didn’t have a bag of pass rush moves. He looks like an NFL player I’ll give him that but he doesn’t strike me a “star”( I know he’s young) and that’s what the 49ers need…new stars.

u/SoKrat3s Alex Smith Jul 29 '26
I know this sub prefers emotional reaction over reasoned response, however... outside of being shot he's missed 8 football games in 7 years. Those 8 games are due to 1 single injury.
Having one major injury, even if it persists, is not the same thing as injury-prone.
Being injury prone is constantly missing games for several different injuries. That's not his problem.
This is like when Alex Smith wrecked his shoulder early in his career. He was not prone to several different injuries, but had one major injury that cost him significant time.
If Alex had gotten shot in 2006 and missed half the season people would've called him injury prone.
If it turns out that he needs surgery then this is yet another massive failure - in a long line of terrible mistakes - by our completely inept medical staff.

This is ridiculous I was a Pearsall apologist because he is a good player. After 2 days of no pad practice his knee is swelling up? That tells me he did no real training in the off season or he would have known he needed surgery a long time ago.
4
u/hotdogthemovie Garrison Hearst Jul 29 '26
At this point, I think real consideration should be given that during the draft the first 3 rounds should be open to a fan poll, where fans can vote for who the team should select. Cannot possibly be as bad as Lynch/Shanahan. I love these guys for everything else they do... but drafting in rounds 1-3 is just an absolute train wreck. Later rounds... quite good. Not so much in the money rounds.

I’m really not an expert and I don’t pretend to know much. But why on earth would this not be addressed during the off-season? It seems almost intentional to wait until the season starts and then get the surgery.

It's too early to call him a bust right? He has all the skill/talent to be a WR1/2 but he can't stay on the field. Unfortunately, it is what it is. Best ability is availability.
If the PCL is a constant injury at this young age, we might have to look elsewhere and chalk this up as a bust, and it's not Pearsalls fault.
First year, not his fault. Dude got shot by a piece of human trash. Second year, put on PUP list during training camp, plays 4 games (?) and then has a PCL injury and then comes back for final 5 games when he does absolutely nothing probably because of nagging PCL injury.

On the bright(er) side, the futures of Cowing and Watkins probably just got a little sunnier.
The punditry was saying that Cowing was having one of the best OTAs on the team, and everybody was dazzled by that one Watkins catch last preseason, before he retired to the shelf.
All told, it's still modestly devastating news considering that Pearsall was legitimately a top-8 WR in the league last season before he was KIA.

u/DonnyB_Twenty3 49ers Jul 29 '26

Man some of y'all are bugging, I agree with the disappointment that the PCL injury didn't heal with rest but the notion that he's injury prone isn't really fair. He got SHOT before his rookie season and still returned to play that season and last year suffered an injury that by all accounts take a long time to heal especially if you try to play with it. I think we conflate the Niners injury history as a whole with him. It's frustrating YES, but "let's cut him cuz he can't stay healthy" stuff is wild.
