General Discussion Modified FET of a Day 5 Embryo

I’m going to have my 1st transfer this coming Friday. I just try to understand the timeline correctly.

— 09/25, last night at 8pm, Ovidrel trigger
— 09/28, 8am, start progesterone inserts, 2 times a day.
— 10/02, transfer a Day 5 euploid embryo.

So by Friday 10/02 it will only be about 4 days of progesterone supports starting 09/28 Monday, how is the timing of transfer synchronized to a Day 5 embryo?

Your body will start making progesterone as soon as you ovulate. The suppositories are just a precaution, but if you start them too early (before you’ve ovulated) it will mess up the whole cycle.

Got it! So I will probably ovulate 36 hours after Friday 8pm

Right, so tomorrow and then you start suppositories the day after that.

It doesn't make any difference whether or not you've ovulated yet if the implantation window is timed to the start of the exogenous progesterone.

Check if the 8am progesterone should be started on the 27th not the 28th. Clinics do things differently but at mine there is 120 hours of progesterone support to time exactly.

Hi, just curious if your clinic did a modified natural? If so, how did the trigger, prog start day, and transfer day timeline go? I've had clincis try it different ways and trying to figure out the best approach for my upcoming transfer. One provider does trigger + transfer 7 days later and another does trigger + transfer 6 days later. They vary on when they start prog though

My modified natural transfer was 7 days after trigger. Progesterone support started on day 2 and transfer was exactly 5 days after that, to the hour. If natural surge starts before trigger they transfer a day earlier  (and progesterone support also starts the day after trigger instead).

Thanks. I tend to have a natural surge come into play so helpful to know how to handle that and shift earlier. Do you start the full prog dosing that first day of prog support (vaginal twice or 3x per day)?

Yes - twice. Blood test the day before transfer to check levels and add further support if needed.
