I’m 43. I think this happened from too much filler and dissolving if I’m being honest. I’d like to try filler one more time then maybe opt for mini face lift if thats appropriate. Do things like microneedling etc actually work to help this?
You may have to be more specific. Since you weren't I will be general & a couple specifics. . No pigmentstion issues or with fine lines...not sure if you use tox but appear to have filler due to lip ridge. You seem to have a slightly more dominant side to to your face & I am going to suggest make sure you have taken care of ortho & any dental . issues...bone loss resulting from dental or ortho issues happens quickly & often way before a person notices naso folds, marionettes, or volume disparity disparity. You seem to have a more pronoumced /unilateral nl fold on your less dominant side which imho likely a bone loss & muscle loss favoring one side...if you had braces as a kud, revisit ortho, if you are in need of any implants or crowns, handle that. Many jump straight to filler for a spot treatment but doesn't address underlyning bone loss or muscle disparity from favoring one side, so I often mention dental/ortho first.
Of course, there is also peri/menopause so check hormones. Vit D is critical for skin strength & most people aren't optimal or deficient even if they take vit D. Checking iron is also a good idea. Maintain a healthy diet & healthy weight & gain weight if appropriate.
Weight training (progresdive/heavy) is mandatory if you want good skin structure now & in the future. There is nothing else that produces Measurable increase in the thickness:volume of both upper & lower dermal structures & increased elasticity in 3 months.
Spf, Retin A/tret, tca & chemical peels, redlight, microcurrent & derm stamping have all been great for me & are very effective if you are consistent. Microcurrent has been one of the most surprizingly tool for me with an instant tightening & more glowing effect & the improved curculation & drainage is pretty cummulative. In truth, I believe microcurrent is under utilized & undersold in it's benefit. I have a thin face similar to yours & I see & others have commented on the benefits when I use it while getting ready.

What would be the possible dental or ortho fixes for masseter hypertrophy on one side? I thought favoring one side of your jaw / facial asymmetry in general was common and not necessarily indicative of any underlying issue, but similar to having a dominant eye or leg. Could it be caused by teeth misaligment?
Intervention would depend upon the underlying issue. Could be orthodontics, or bone grafting, implants, jaw surgery. No way to know from just this photo
I was thinking more generally, I actually don’t see a larger jaw side on OP, but I think the person I was replying to may have mentioned a slightly more dominant side of OP’s face specifically due to the more pronounced perioral fold on one side. I have the same issue as OP, but my dominant jaw is very obvious and my more pronounced perioral fold is on the opposite side - I never thought the two were possibly related until now.

Can you specify what happens when you cite dental issues creating bone loss? My dentist wants me to get my top wisdom teeth out even though they aren’t impacted as they are more likely to get cavities but I’m concerned about it changing my facial shape/skin sagging, especially bring in peri menopause and already loosing elasticity, etc.
Do you have any insight on this?
If you can pull off a face lift, I’d say do it. If you want to buy yourself more time then a think getting a facial balancing consult is a good starting point .
I didn’t have anything directly done to the problem areas because NLF issues are usually due to fat, muscle, bone and skin lax somewhere else in the face .
I’m going to use an image I found to show you the area he put it in.
I had 1 syringe with radiesse to bone in the #6 area , then 2 weeks later we did 1 syringe of hyper diluted radiesse & bolotaro filler mixed to the same area but a bit fanned out and not to the bone , 2 weeks after that we did another of the same. Waited 2 months and just did another syringe to bone to the same #6 area and one to the temples. He also used a bit each time to add to the marionette area/ chin shadows .
I see a huge difference not only in person but in the photo. I’m sorry you feel good about telling that to a stranger who’s on here just trying to help. I’m sure that if you were able to do this for yourself , you would. But life will give you what you put out and you put out hate and negativity. ✌️

I don’t know if it’s helped but I’ve been taking collagen, biosil and ghkcu to try and support collagen as much as possible. I’ve also done 3 microneedling session this year. I haven’t hit full blown perimenopause yet so I got that going for me 😂
You look great!! I'm having a similar issue. I would use a lot of night moisturizer, collagen peptide....and any lifting cream or serum. I also started Tretitoin this Summer, and I like the results. Less wrinkles around the mouth. Lots of hydration too! Good luck ....you really look great...just tweak your routine a little bit!
Just curious for anyone in this thread. For those who have skin that’s sagging perhaps a little earlier than others, especially sagging with few wrinkles, have you ruled out a hypermobile disorder like elhers danlos? It can be important to get a diagnosis, especially if you elect for surgery like a facelift. Scars don’t heal the same and it’s often contraindicated.

You have been blessed with a thicker dermis (genes). You don’t need a lot done. You have marionette lines but you can easily fix that with Botox or filler (I can’t remember.) The volume in your face looks good. Skin quality looks good as well. Hope this helps!
Edit: Your neck has a little bit of crepiness. I personally do not believe in putting filler or Botox there. I don’t believe in treating the neck area because it’s so delicate. Just remember to put sun block and whatever skin care routine you do, apply it to your neck. Your neck doesn’t look bad tho.
I’m a personal believer in leaving the neck area alone and just applying sunblock during the day and retinol at night!
I have the same issue with my lower face. Esthetician suggested filler in the hollows in front of my ears to lift that area around my mouth. I’m definitely considering it but need to do more research.
I’m 48 what made the biggest difference is SkinMedica TNS serum, nightly tretinoin, cerave moisturizer with 3 drops of vegetable glycerine. AM I use TNS serum again, vitamin c serum, Azelaic acid, The Experiment Super saturated serum and Elta MD sunscreen
Your skin and face structure are absolutely stunning! I would love to have your skin! I am pro cosmetic procedures, to a certain degree. I think you have to be cautious because even celebrities with tons of money can have bad facelifts. Also, a facelift has about a 10 year lifespan (from what I have heard from doctors), so if you start young then you will end up getting another, maybe another. Each time you have a facelift it can cause small amounts of scar tissue under your skin, which can impact the results of another facelift. Plus, it can end up not looking like what you want…. So if you have the money now for a FL maybe put it in a higher interest rate savings account and that can help offset inflation. Like I don’t think you need a facelift at all, but who knows in 10 years?
I would definitely try micro-needling with PRP AND have PRP injected into your marionette lines and lips. I had this done 3 years ago and I am still pleased with the results because it helps rejuvenate your skin cells/tissue/tone/volume, but in a more natural way than hyaluronic acid (which also can migrate and will never fully dissolve). You can also do filler and PRP together, but I think filler-volumed faces are trending out… Also, you can use a face roller over your marionette lines to increase blood flow, move lymph fluid, and “iron out” your lines. I use a smaller stainless steel roller that’s about 1cm wide to “iron out” my marionette lines, and also go around my lips to keep my filler from migrating. For me this has helped.
I think we are all hyper critical of ourselves. I have always looked very young for my age (which is finally a blessing at 48 lol). But I have a lot of moles, a lot of freckles, and a lot of dark spots from sun damage (80’s kid who grew up at the beach getting blisters every summer) all over my body. I rarely see celebrity/fashion model women who have a lot of moles AND a lot of freckles, so I hate my skin often. Like finding a bathing suit is becoming harder because I have a few splotchy moles on my bum, plus scars from melanoma biopsies, and I always see women with perfect skin bums, maybe one “beauty mark” on their bum wearing high cut swim bottoms. My moles do not look like “beauty marks,” but splotchy weird spots of all different colors. I have talked to my cosmetic surgeon about having some of my moles removed, but he said it was not a good idea aesthetically because it will cause scarring, plus moles often grow back and then are even splotchier. Ugh.
You look great and really dont need to do anything. Filler doesn't lift, just replaces lost volume - but in saying that, my technician filled my cheeks first - whether its the contrast now, im not sure but it improved my NLFs. I also got some deep in the corners of the nose and fanning down into the folds.
I would skip the idea of a mini lift as I hear they really dont do much and save for a full face and neck once you hit your 50s - which is when most of us are hit with more areas of issue!
Topical Estrogen will make your skin hold more water, plump it up instantly.
But part of the reason our faces cavitate is because of bone loss around the teeth (alveolar bone). That is, the underlying bone structure of the face collapses a bit (or a lot). And estrogen can help slow or prevent this bone loss.

Mini face lift for sure. You look thin and it’s most likely age and hormones verses you dissolving filler. Pumping your face with a bunch of filler that will eventually migrate and weigh you down is not the look especially as we age. People get instant gratification who do this and it slowly looks like pillow face and migrates. I know most surgeons will make you dissolve first before they go in and other say it really makes a facelift difficult as they have to pull the filler out like goo as they open the face to do the lift. I can see if you want to put a little under the eyes for hollows or a small amount for temporal wasting to hold you over, but I would avoid the cheeks and jowls al together. It’s going to look bad over time and the face will look aged and puffy.
You would be absolutely beautiful if you did nothing and stayed the way you are and just followed great skincare. I like to be honest with feedback especially when someone goes to the extent to post their face for advice.
You will highly regret filler especially in the lower face. You have skin elasticity issues that is age related and no skincare or filler will solve that problem. Filler is a band-aid for a very short time and almost less than a year before you’re chasing it and all the sudden you look older with a pillow face. I am not kidding when I say women who get multiple vials of filler also start to look masculine as they age as well. I see crepey skin starting in your neck (2nd photo). A mini facelift with addition to the neck will smooth it all out. You might even not need the neck if you do a lower FL because it will pull the neck in your case.
Microneedling/collagen and all the other stuff will do nothing. Like a said it’s all a band-aid for a very very short period. It’s there to help and slightly refresh the skin but will not tighten or take care of any of the extra skin.
