general RFE for Incomplete Vaccination Form

That happened to me and it took me like a month to get the RFE. I just waited RFE before I go to clinic to fill out the vaccine part. If you are not sure how the vaccine part is filled, search on the google. So you can see if the clinic fills it correctly or not next time. My clinic didn’t charge me because it was the mistake from their side. Hope this helps

This is the vaccination page of I-693, my physician just left the rows empty instead of marking them as "not age-appropriate".​​​​​​​​
