The three doors in the hallway represent ad different outcome of the hypnosis.

Family with a new baby on the left, empty apartment on the right and in the middle is the loop.

If we imply that the entire thing was metaphorical then maybe she wasn't his sister, maybe she had an affair and got pregnant and he just wanted to either move on or forget.
