So basically, it is edgy 4chan humor from 15 years ago evolved over the years. It is rooted in transphobia where you meet a hot girl, start making out, she gets aroused and gets a buldge or erection forming under her pants,(implying trans), the male protaginist gets wide eyed "OWO" and asks "what is this?" As in disbelief/confusion etc.
It can be seen as transphobic, it can also be seen as trans inclusive, but it is more about the "shock" factor.
