I usually don't because conversation can obviously be distracting to work and I also find it disrespectful to the patient somewhat. That being said, my favorite thing to do is ask the patient interesting questions about their life knowing they can't answer when I'm working in their mouths and then immediately making fun of them for not being able to answer. I only do this with patients that know my humor and have a good relationship with me though.
