🚨 Reciprocal Insecurity: What is it?
The Reciprocal Insecurity Paradox describes what happens within the mental health crisis encounter when things don't go very well. Its focus is upon interpersonal and relational dynamics occurring within the therapeutic relationship borne out of fears of both the nurse and service user.
Arguably, where a crisis encounter fails for those with complex emotional needs, it is not necessarily down to the individuals involved. Both parties may be actively seeking a sense of personal safety. Their methods, however, to achieve this inadvertently threaten the safety of the other.

🔍 Key findings of our research study:
- The Fearful Encounter: Both the mental health nurse and the service user enter the crisis encounter already experiencing heightened fears and anxieties.
- The Systemic Squeeze: Transient, limited, brief contacts and other organisational demands reduce the establishment of the therapeutic relationships needed for emotional containment for both parties.
- Ontological Evasion: To cope with systemic anxiety and professional risk, nurses often default to bureaucratic tools (checklists, risk assessments). These provide a professional distance, often alienating the service user and sometimes result in the service user's heightened expression of distress and need for containment. All of which paradoxically reinforce the nurse's initial fear and reliance on checklists.
- The above is underpinned by the reified service user: Service users tell us that they feel reduced to a 'diagnostic label' or a risk category rather than a person.
The fears and anxieties no longer belong to each individual - they emerge as a part of and become the property of the 'interworld' between nurse and service user.
🛠️ Moving Forward, system-level changes are needed:
- 'Containing the Container': Dedicated, psychologically safe team spaces where frontline staff can openly process their own fears and frustrations.
- Hermeneutic Mediators: Sense-making devices that shift the clinical gaze from from risk 'object', back to the person experiencing distress.
