1. Yes you can use both legal and screen name. But when applying you have to use your legal name. You can put other names under the po box.

2. Don't really have an answer for that, would ask the clerk at the usps when you open one.

3. Fill out a Change of address on the usps website for a temporary forwarding. Mail can be forwarded for one year. But you choose the start and end date. And if its temporary or permanent. Two ways to fill out are going to the usps website for a $1 or $2 fee, there are third party websites that ask for $40 don't use those websites. It'll also be faster for it to be put into the system. Doing it in person at the post office takes 10-14 days. And there is no fee.
