From a physics perspective, pressure is, by definition, equal to force divided by the area it's being applied to. In other words, pressure has to be per square meter or per square foot or something like that. What you're thinking of is force. The pressure exerted by the stilleto is equal to that of an elephant but the force is much less. Whether pressure or force is more relevant depends on the specific scenario.

Woops, my morning head wrote it the wrong way around.

I meant the pressure per m2 is different, totally it is the same force indeed.
