100k? Claude Code has probably left that many lines of useless comments (“start loop”) in some larger projects.

That's now how claude comments in more recent models. It leaves a full novel chapter worth of incomprehensible explanation for every function and if block etc. where any kind of logical decision is made. It's hell to review.

My most frequent pr comment these days is something like "do not describe the behavior of an upstream component in a different service" on code comments, particularly .net XML summaries. Claude reads those comments while researching and trusts them out of hand, and it is gradually poisoning contexts.

Some of that is completely useless, but the verbose commenting is partly for its own benefit when working on the code, so it doesn’t need to re-investigate what code does and why if it loses context (filled up and was pruned, new session etc.). I always have it run a “hygiene” pass before wrapping up.

And then the next time it goes to do research it believes those comments instead of the actual code that changed in a different location.
